Dr. Robert Jantz
Dr. Jantz is a board-certified family physician who has been in private practice since 1984. He graduated from Temple Medical School in Philadelphia, PA, and completed his residency in Indianapolis, Indiana. He moved to Lebanon, Tennessee, in 1988 and worked as a family physician in a group setting before opening his own private practice in 1993.
He is well-versed in treating all types of common illnesses found in family practice and treating more complex problems such as diabetes management and hypertension care. He also performs minor surgical procedures in office on skin cancers, cysts, and the removal of skin tags and warts.
He provides personalized, comprehensive health care, focusing on wellness and prevention.
Steven Pack, Physician Assistant
Steven is board-certified by the National Commission on Certification of Physician Assistants.
He received his degree from Trevecca Nazarene University, where he graduated Magna Cum Laude in 1997.
Steven has been practicing medicine for over 20 years and has experience in Emergency Medicine, Cardiology, Internal Medicine, Cardiology, Internal Medicine, Hospital Medicine, and Family Practice.
He is dedicated to providing quality and compassionate care to his patients and the community.
He resides with his wife, LeaBeth, and their three children in Lebanon, Tennessee.
LeaBeth Macon Pack, Nurse Practitioner
LeaBeth Pack is certified by the American Academy of Nurse Practitioner as a Family Nurse Practitioner.
She earned her Bachelors of Science in Nursing at Middle Tennessee State University and her Masters of Science in Nursing at Tennessee States University in 2010. She began her career as a registered nurse in the Cardiac Intensive Care Unit at St Thomas Hospital in Nashville, TN.
Her specialties include heart transplant, LVAD training, Coronary Artery Bypass, and Neurology Intensive Care.
LeaBeth’s prior experience is predominately in the acute and intensive care setting for 15 years. During her clinical rotations, she gained experience in primary care, emergency room, women’s health, cardiology, and diabetes treatment and management.
Leabeth believes in providing the same care to her patients that she would expect to be given to her family. She is very dedicated to building an open and trusting relationship between the patient and provider. She spends time thoroughly listening to patients and strives to help them make informed, positive health choices.
